Heart failure is recognized as the inability of the heart to maintain a balanced circulatory channel in the body and is often a result of a congestion or constriction of the vessels (Amakali, 2015). Cardiovascular disease prevention and readmission management is not always possible in primary care settings; hence it is vital that the nursing professional is able to adopt a chronic care and management model for effective nursing intervention for the cardiovascular patients. A nursing professional must design a clinical care plan that reduces patient' noncompliance, and hence reduces their extent of readmission in the hospital settings.


Aim of the paper

The purpose of this review was to understand the reason for the noncompliance to the treatment amidst the patients with cardiovascular diseases (CVD), and examine what education techniques that can help in preventing their readmissions to hospitals. 

Objective of the paper

The present paper provides a brief summation of the central challenges faced by the nursing professionals in managing CVD patient compliance. The competent care for the patients with heart failure requires comprehensive assessment of the patient challenges and an understanding of their disease pathophysiology (Lalonde et al. 2012). In this sense, the present research helps in comprehending the scope of evidence based practice for the nursing professionals to improve patient outcome through the use of multidisciplinary educational intervention approach.
