What are the impact of Holistic and Person-Centered Approaches to the Community-Based Nursing Care Patient Outcomes?

Introduction

Patient-centredness is based on the holistic assessment of patient experiences regarding their disease pattern, healthcare challenges and individualized requirements. The administration of holistic and patient-care procedures not only control the healthcare cost but also improve patient satisfaction and health outcomes across the treatment environment. Person-centered care diversity and individualized preferences of the treated patients in the healthcare environment (VSG, 2015). The need-based health care choices and decisions through patient-centered care substantially minimize the length of patient stay in the treatment setting while concomitantly improving the wellness outcomes. The holistic and person-centered environment promotes the pattern of nurse-patient interaction while increasing the scope of patient clarification and complaints regarding the provided treatment. 

Patient-centredness in community-based interventions has been a matter of debate across the healthcare sector. The complex health care requirements of the chronically and seriously ill patients substantiate the need for interdisciplinary collaboration of treatment providers, clinicians, nurses, and patient's family members to effectively improve the patient care experience and wellness outcomes (IOM, 2013). Community health assessment through person-centered approaches is highly necessary with the objective of improving the patient care environment and promoting resilience in the diverse patient population. Community-based healthcare programs effectively improve the overall well-being of patients pertaining to various age groups, societal domains, socioeconomic strata, and geographical regions. The person-centered and community-based healthcare approaches systematically promote the utilization of motivational interventions for the meaningful assessment of patients' complex health care needs (Fooladi, 2015). The assessment by (Kim & Park, 2017) indicates the effectiveness of person-centered approaches on the mental health and wellness of dementia patients. Indeed, the incorporation of person-centeredness in community-based mental health care approaches substantially improves the depression pattern, neuropsychiatric manifestations, agitation level, and quality of life of the mentally ill patients. 

Person-centered philosophy of care advocates the need for patient empowerment through the utilization of social stimulation and environmental enhancement strategies. Person-centered community-based care focuses on patients' physiological and safety concerns that require prioritized resolution before addressing the elevated level requirements (Brownie & Nancarrow, 2013). Person-centered community-based healthcare approaches effectively increase patients' access to the available diagnostic and treatment modalities in the hospital setting (Randall, Crawford, Currie, River, & Betihavas, 2017). Person-centered community-based approaches elevate the level of self-sufficiency, self-care, and self-management level of the chronically ill patients to a considerable extent (Massimi, et al., 2017). This indicates the person-centered interventions effectively the level of patients' autonomy and capacity to undertake the activities of daily living and personal care. Indeed, the presented evidence-based findings reveal a range of beneficial outcomes of integrating the patient-centered approaches to community-based nursing care interventions. The presented research proposal will effectively reaffirm the advantages of person-centered nursing care approaches on the healthcare outcomes of the patients who continue to receive community-based interventions in various hospital settings. The research study will utilize expanded parameters to understand the true worth of integrating person-centredness in a range of community-based interventions across the treatment environment. The proposed research study will address the following questions. 

1. Do the patient care approaches empower the chronically ill patients to modify their lifestyle and overall well-being in the healthcare setting?
2. To what extent the person-centered healthcare strategies address the individualized requirements and treatment challenges of the patient population?
3. To what extent person-centredness in community-based nursing interventions assists in improving the mental health outcomes of the treated patients? 
4. Do the person-centered community-based healthcare approaches enhance the informed choices of patients and improve their recovery time?
5. Do the person-centered community-based approaches improve patients' healthcare transitions and advance care planning in the treatment facilities? 
6. Does the incorporation of person-centeredness in community-based nursing care interventions improve patients' treatment compliance and response rate in the clinical setting?
7. Do the person-centered community-based nursing care strategies enhance healthcare transparency, equity, accessibility, and duration of patient stay in the treatment facility?
8. To what extent the person-centeredness of the community-based nursing interventions promote patients' safety across the treatment environment?
9. To what extent person-centered and community-based care safeguards the rights and dignity of the treated patients (elderly patient in particular)?
10. Does the incorporation of person-centeredness in community-based interventions promote the process of shared decision-making and safeguard the rights and dignity of the treated patients?      

The recommended research intervention will effectively evaluate patients' healthcare perceptions, experience, outlook, opinions, expectations, and responsiveness in relation to the impact of holistic and person-centered nursing care approaches on community-based nursing care outcomes. The assessment of patients' perceptions is a significant attribute of qualitative research design (Samsson, Bernhardsson, & Larsson, 2017). Furthermore, qualitative intervention is the recommended research strategy that facilitates the systematic interpretation of participants' understanding, feelings, behavior, and thoughts regarding the phenomenon of interest. The recommended research study will, therefore, deploy a qualitative research design based on the research questions, objective, and hypothesis.
